Marlton office. I am deeply concerned by the severe inaccuracies in my medical charts and the defensive reaction of this practice. My visit summary falsely documented that I had 'gait instability,' a 'headache,' and 'peripheral edema'—none of which I have ever had or discussed. It also listed code I11.9 (Hypertensive heart disease), completely contradicting what I was told verbally during my appointment. A single swollen vein in my thigh was apparently coded as peripheral edema without anyone mentioning it to me. Furthermore, the doctor would not let me finish my sentences during my face-to-face appointment. When I reached out to ask why my charts were grossly incorrect, he again refused to let me finish my sentences over the phone, and told me I should find another cardiologist. Patients have a legal right to accurate medical records. Being terminated from a practice simply for questioning terrifying and incorrect diagnoses on your own chart is unacceptable. I cannot recommend a practice that handles patient communication and record errors this way.
