If you believe that The Heart House and Vascular Care has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with our office. Call (856) 546-3003 (TTY: 711) and ask for the practice administrator. You can file in person, by mail, by fax, or by phone, and we can help you complete the grievance.
File a complaint with the U.S. government
You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through the Office for Civil Rights Complaint Portal, or by mail or phone. Complaint forms are available online.
Language assistance
